General Summary
The Care Manager collaborates with members of the healthcare team, community resources, and with the patient/patient family to promote optimal outcomes for the patient across the continuum of care. The Care Manager provides Care Management activities to ensure an effective, appropriate, individualized and safe plan of care. The position is goal-oriented with a high level of accountability correlated to the continuum of care, quality of care and patient satisfaction.

Qualifications
Education: Graduate of an accredited college with a degree in social work and current licensure in the State of Maryland. Master's degree with LGSW or LCSW certification preferred.
Experience: Minimum of 3-5 years of relevant clinical hospital experience preferred as applicable to the position needs. Ongoing education, certification, and self-development to remain current with industry standards and business objectives related to hospital case management.
