The Intensive Foster Care program (IFC) seeks a Clinical Case Manager to join their team based out of the Dorchester offices. The IFC Program provides transitional foster homes for children and adolescents with emotional and behavioral issues referred by DCF Department of Children and Families. IFC trains specialized, intensive and therapeutic foster families who can provide the special care these children need and help them through the transition back to their families or a permanent kinship, guardianship or adoptive placement.

Permanency provides a child with a legal and emotional connection to a family, however "family" may be defined.

Under the direction of the Program Director, the CCM will provide individual and family based therapeutic services, crisis intervention and case management; provide counseling and therapeutic support to clients; coordinate appropriate resources and services for clients and families; act as an advocate for the client; administer written assessments, summary reports, documentation, treatment planning, and safety planning to appropriate parties in a timely, well written manner; submit reports, and documentation in a timely manner. Ensure that documentation meets utilization review and quality assurance standards; participate in an on-call rotation.

Qualifications

  • Masters Degree in Social Work or Psychology required.
  • Ability to obtain licensure within 90 days of hire and/or already licensed.
  • Having experience with behavior management systems, treatment planning, crisis intervention, case management and providing some individual, group and/or family therapy is important with this position.
  • Excellent written and verbal communication skills are imperative.
  • Knowledge of Department of Children and Families and other local child and family advocate agencies is vital.
  • Must have a vehicle to attend meetings, home visits, and transport clients to appointments.
  • Bilingual in Spanish/Cape Verdean Creole or Haitian Creole preferred.
