The Community Health Worker is a frontline public health worker. This position serves as a liaison between residents, health providers and social service agencies in the community. The Community Health Worker helps facilitate residents' access to health and social services to improve their health outcomes and wellbeing. Other responsibilities include helping build residents' health knowledge and self-sufficiency through a range of activities such as outreach, education and coaching, social support, and advocacy.

Responsibilities Include:

  • Collaborating with Resident Service Coordinator to identify and help address resident needs.
  • Assisting residents with identifying issues that affect their overall health and developing health/social management goals and plans.
  • Linking residents to needed health and wellness information, resources, and services and providing follow-up support.
  • Translating and interpreting health information for residents.
  • Assisting residents with communicating issues and concerns to their care providers and helping to build their capacity for communication and self-advocacy with providers.
  • Developing and/or coordinating on-site health and wellness programs.
  • Engaging residents in on-site health and wellness activities.
  • Supporting residents during/following hospital stays and/or doctor's appointments.
  • Facilitating health education and behavior change strategies with individuals and groups.

Required Qualifications:
  • 2+ yrs experience and/or education in health or social services environment
  • Ability to establish trust, listen, and assist residents with defining their health-management issues, while helping them identify possible solutions
  • Ability to problem solve, advocate, and provide results for residents served
  • Strong interpersonal and verbal/written communication skills
  • Ability to work collaboratively with colleagues and other stakeholders
  • Ability to work independently and self-manage
  • Proficiency with Microsoft Office (Word, Excel, Outlook) and ability to learn electronic documentation systems
Preferred Qualifications:
  • 3+ yrs experience and/or education in health or social services environment
  • Knowledge of community health resources, services and health insurers
  • Knowledge of health-related issues of older adults, such as physical and mental impairments common to the aging process
  • Experience working with older adults in a direct service capacity
  • Knowledge of Medicare, Medicaid, and other public benefit programs
  • Completion of a Community Health Workers certificate / credentialing program
  • Bilingual in English and Spanish
