Job Title: Hospice Case Manager RN
Details: 13-week assignment with the potential to extend. Days currently available, would be 5x8hr shifts/week Monday-Friday.
Minimum Requirements:
- Active CT or compact RN license
- Hospice experience required
- HCHB and OASIS experience preferred
