Outstanding opportunity exists for a Registered Nurse to become part of our new Community Partner Program! The Community Partner Program is an innovative program that integrates behavioral health with physical health and focuses on "whole person wellness"!
The RN is a critical member of the Community Partnership (CP) Team who works closely with the Clinical Care Manager to supervise Care Coordinators and ensure comprehensive care coordination for engaged enrollees. As part of the CP Team, the RN will review all engaged enrollees medical history, medical needs, and medications prior to the Comprehensive Assessment being approved by the Clinical Care Manager. The RN is responsible for medication reconciliation for all engaged enrollees discharged from an emergency department visit and/or inpatient hospitalization. The RN will provide clinical oversight to help the CP Team understand and manage patient's co-occurring medical conditions.
Required Skills
- Excellent communication skills and ability to work successfully and collaboratively with internal and external contacts
- Ability to assess situations and use appropriate independent judgment to determine most appropriate course of action including during critical/urgent situations
- Time management and ability to effectively organize priorities
Required Experience
- Minimum of Associate's degree in Nursing, Bachelor's degree preferred
- Valid Massachusetts Registered Nurse licensure
- Previous experience in providing care coordination for high risk individuals
- Valid driver's license, safe driving record, and access to personal vehicle for outreach and to provide client transportation as needed
