Summary:
The Population Health Manager will oversee clinically integrated care that supports whole person care, value-based care initiatives and reports to the Population Health Service Organization or Memorial Primary Care leadership.
Responsibilities:
Provides clinical expertise, clinical judgement, and case management interventions consistent with patient-family centered care in the ambulatory setting. Source and develop evidence-based training material, presentations, and share readmission review case studies to promote best practices and effectual clinical decision-making and mobilization of patient resources.
Identify process improvement opportunities that foster a shared effort from multidisciplinary teams across the organization in alignment with value-based initiatives.
Orients, trains and supervises new care team members or when processes change including but not limited to review of clinical workflows, performance expectations, policies and procedures and training manuals. Routinely produces communication as needed to satisfy educational and training requirements for changes in policies and procedures.
Establish and maintain a relationship with clinical effectiveness team members, hospitalists, discharge coordinators, community providers, and post-acute providers.
Consistent application of effective team leadership, mentoring and coaching. Managing team performance including coordination, supervision and development of clinical protocols and daily operations.
Conducts employee performance evaluations and provides leadership development, mentorship, and formal coaching. Assist in development of short-term and long-term department goals in alignment with whole person care, patient and family centered care, and value-based care strategic goals and objectives.
Ability to work cohesively in a collaborative leadership role with care team members, providers, and medical support staff to articulate complex concepts in managed care and coordinated care principles.
Represent Memorial Healthcare System in leadership strategic meetings with internal and external shareholders. Acts a resource and liaison with community and post-acute providers. Works collaboratively to develop cross continuum clinical collaboration.
