Description:
Outreach to members to complete a Comprehensive assessment to identify gaps in care and any barriers, identifies the most appropriate resources and or support systems that will be included as part of the member's multidisciplinary team, uses evidence-based clinical policies and guidelines as the framework for clinical decisions and service provision that optimizes member care and outcomes, focuses on the reduction of avoidable unplanned hospital admissions, impactable medical expense and reducing the burden of illness for members with complex or acute medical and behavioral conditions.
